As York is a "green" school, and to greatly aid collection and streamlining of requested materials, we utilize the online Magnus Health Student Medical Record (SMR) system for collecting student health information and school registration materials. The health and safety of York students are the highest priority for York School.  Utilizing Magnus allows the School to collect and maintain this information in a secure and convenient manner, as well as provides secure access to student information in the event of an emergency. Parents are encouraged to visit magnushealth.com/parents to review the welcome packet provided by Magnus Health SMR.

Shortly after enrollment contracts are received, parents will have access to the student Magnus Health SMR account(s) which will be accessible via the "Resources" tab in myYork. Required information consists of online entry and a combination of paper documents that must be signed with original signatures and uploaded into the account. 

Note on Immunizations: There are a number of health documents included with registration that may or may not apply to incoming students. However, California Law requires all students be fully immunized; please click here to see a list of required vaccinations. A copy of each student's full vaccination record is required and should be uploaded into the Magnus account no later than July 15; this is required prior to school entry.

  • Usernames and Passwords

    Magnus Health has been integrated with myYork and can be accessed via the myYork portal > Resources tab > Magnus Health tile.
  • Initial and Renewing Registration

    The Magnus student account is where parents will directly enter online or upload important information such as health history, and other required registration information.  Please allow 30+ minutes to complete (for new students) or review (for returning students) the online Vital Health portion of the registration. All other documents must be downloaded, printed, reviewed carefully, *signed by the student and/or parent (and if applicable in some cases, a medical provider) and uploaded directly into the Magnus account.

    Note: Emergency Contact information entered in myYork will automatically sync to the student Magnus Health account. Parents can update emergency contact information via myYork > Profile tab (upper right hand corner name with dropdown).
  • Original Signatures

    Original signatures on initially submitted documents are required. Documents substituting typeface/font (such as in fill-in Adobe forms) or JPG/GIF images for the signatures will be rejected and tagged for re-submittal. Returning student registration will allow for simple review and one-click agreement on several of the requirements.
  • Physical Examinations

    With changing circumstances and needs it is encouraged that every student have a valid physical exam in the Magnus account prior to participating in all York-related activities. All Physical Examinations must be signed and dated with the actual "exam date" of the exam by the student's medical provider and are valid one year from the date of the exam.

    Note for Student-Athletes: CCS bylaws require all student-athletes intending to participate in school-related sports to have a current/valid physical examination on file with the school. Only student-athletes with a current and valid physical exam on file will be allowed to participate in York-related sports events (e.g. practices, games, etc.)
  • California School Immunization Law

    As of 2016, California law mandates that every student must be fully immunized.* To comply with California law regarding vaccination requirements, a complete immunization record for the student must be uploaded; either a copy of the record you have in your possession (complete with medical provider stamps or signatures) or a transcript of immunizations from a medical provider's office. Please see the following helpful links for more information. 
    Immunizations for domestic students must be complete or actively in progress (i.e. "catchup plan") and part of the student account no later than July 15 to ensure compliance prior to school entry. 

    Immunizations for international students must be fully complete prior to the first day of school – no catchup plan is allowed.

    *Medical Exemptions

    Starting January 1, 2021:
    • Medical exemptions can only be issued through the California Immunization Registry – Medical Exemption website (CAIR-ME) by physicians licensed in California. 
    • Schools and child care facilities may only accept from parents new medical exemptions that are issued using CAIR-ME.
       
    Medical exemptions filed for children in attendance at a school before 2021 may be valid until the earliest of:
    • When the child enrolls in the next grade span (TK/K-6th grade, 7th-12th grade)
    • The expiration date specified in a temporary medical exemption
    • Revocation of the exemption because the issuing physician has been subject to disciplinary action from the physician’s licensing entity.
  • Weekly Alerts

    Automated weekly alerts are sent from the Magnus system as a reminder of any outstanding registration requirements. This serves as a helpful reminder to families about items pending or requiring completion. If you have more than one child at York, you will receive multiple email reminders.
  • Submitting Registration Material

    If a student is returning, some of the documents already uploaded into the Magnus account are as easy as a one-click digital authorization indicating agreement with requirement. Just review the document again so there is an understanding of what that requirement involves and it can be self-approved. 
    • Scan and Upload Directly Into Magnus Account - Processing time: Immediate
      The most expeditious way to ensure your registration paperwork is submitted and marked as complete and/or reviewed promptly is to print each document, sign, and utilize the "Scan and Upload" feature for each requirement. Utilizing your printer, scanner, or mobile device to upload documents expedites completion of the student registration.  
    • Fax to Magnus - Processing time: 3-4 business days with cover pages; 7-10 business days without.
      You may also print, sign, and utilize the "Fax or Mail" feature, submitting the forms directly to Magnus Health. When using this method, please be sure to print the cover page for each document as it contains a unique barcode that enables correct processing into the student record. Documents submitted with this cover page may be processed within 3-4 business days; without this cover page, processing will be delayed. Magnus' fax number: (877) 447-9530. 
    • *Requirements submitted directly to York School can not be accepted and can not be uploaded on your behalf. 
    All materials are due July 15.
